DOCUMENTATION AND REQUIREMENTS

1. Proof of Citizenship

To replace a lost or stolen passport, applicant must bring an original or certified copy of a birth certificate, naturalization papers or old passport.
Birth certificates must show applicant and parent(s) full names (“short form” birth certificates may not be accepted).

2. Proof of Identification

Applicant must have a driver’s license or state-issued identification issued more than six months ago to apply for a new passport. Official government or military picture identification is acceptable.

3. Passport Application (DS-11)

Applicant must submit one completed passport application form (DS-11) by following the link below. The U.S. State Department website “wizard” will walk you through the application. After completing the passport application online, print by pressing the “Create Form” button. Print two (2) completed passport applications. One copy will be included in the envelope sealed by the U.S. Postal Service Acceptance Agent and the second copy (signature not required) should be included with documents delivered to Smart Visas and Passport Services. DO NOT SIGN THE COMPLETED APPLICATION UNTIL INSTRUCTED TO DO SO BY THE ACCEPTANCE AGENT.

Note: While completing the application, the wizard will prompt you to select fees, shipping and processing options. Select any options to complete form. No payments will be made and options selected will have no affect on the actual service delivered by Smart Visas and Passport Services. The guide is simply to assist the general public in calculating fees for traditional mail-in applications.

4. Missing Passport Explanation Form (DS-64)

Applicant must submit the Missing Passport Explanation Form describing the circumstances regarding the loss or theft. Answer the questions clearly and concisely as if speaking directly to a U.S. State Department official. Questions regarding previous passport number, expiration date, etc. may be left blank if the information is not available.

5. Passport Photos

Two recent 2″ x 2″ passport photos of the applicant is required: Please click here for photos guidelines or to take a photo using your smartphone and submit it online.

6. Proof of Travel

Applicant must submit a computer-generated itinerary from a travel agency or online booking receipt (e-confirmation from an airline, Travelocity, Orbitz, etc) detailing departure from the U.S. Print two copies of your proof of travel; one copy will be included in the sealed envelope delivered to the passport agency, the other should be delivered to Smart Visas and Passport Services. If traveling on business and no itinerary is available, a letter on company letterhead indicating dates and purpose of travel MAY suffice. If the applicant does not possess proper itinerary, please contact us for options.

7. Letter of Authorization

Applicant must provide a letter authorizing Smart Visas and Passport Services to handle the documentation requirements on your behalf. To complete the form letter, click on the link: Letter of Authorization

8. Government Fees

The U.S. State Department charges USD 170.00 for expediting passports and USD 25.00 for the verification of new passport applications (by the passport agent). Both fees must be paid to the Acceptance Agent onsite. The New Passport Fee of USD 170.00 may be paid by check, cash or money order, and will be enclosed in your sealed application envelope. The Acceptance Agent fee of USD 25 may be paid by any payment method.

Sealed Envelope Requirements: Bring all of the requirements listed above to the post office. (Check your post office to see if they have a Passport Agent on dut and if they require an appointment for expediting passport applications.) Once at the post office, the Passport Agent will review your DS-11 application, make sure all your required paperwork is in order, and guide you through an Oath & Signature to verify your identity. The agent will enclose all your documentation inside a U.S. Postal envelope and seal it with an official US postal stamp.[/vc_column_text][vc_column_text]

SHIPPING INSTRUCTIONS

The outside of the sealed envelope (handed to you by the Acceptance Agent) should be clearly marked as follows:

Last Name, First Name
Date of Birth
Departure Date
Contact Phone Number

Note: If there is more than one application in the envelope (for multiple applicants), write out each applicants name and date of birth.

Applicant should place the sealed envelope from the post office passport agent (clearly marked as instructed above) together with the Smart Visas service fee (see below for a list of fees) and the list of documents outlined below in a FEDEX envelope and send to Smart Visas and Passport Services:

  • Extra copy of travel itinerary
  • Extra copy of original signed Letter of Authorization (marked “EXTRA COPY”)
  • Extra copy of completed passport application (signature not required)
